Influence of fracture type and thickness of intramedullary nails on fracture healing – A numerical study

چکیده

INTRODUCTION: It is common knowledge that the fracture healing process is strongly influenced by the mechanical behaviour of the bone implant complex (BIC) due to the resulting interfragmentary movement (IFM) of the bone fragments at the fracture site. Unreamed intramedullary nailing reduces the disturbance of blood supply in comparison to reamed nailing but leads to a more flexible stabilization due to the distance between nail and endosteal surface. Therefore high interfragmentary movements (IFM) may occur under low shear loads which could result in a delayed fracture healing. To identify the mechanical key factors influencing the healing outcome of intramedullary nailing, we simulated the fracture healing process for typical types of diaphyseal fractures treated with intramedullary nails of various diameters.

Treatment of Pediatric Open Femoral Fractures with External Fixator Versus Flexible Intramedullary Nails

Background: In children, inappropriate treatment of open femoral fractures may induce several complications. A few studies have compared the external fixator with flexible intramedullary nails in high-grade open femoral fractures of children. The present study aims at comparing results of these two treatment methods in open femoral fractures. Methods: In this descriptive analytical study, 27 ...

The Influence of Using One or Two Lag Screws on the Mechanical Environment of a Femoral Neck Fracture

Fracture healing requires two major pre-requisites; sufficient blood supply and mechanical stability [1-2]. Optimum mechanical conditions for fracture healing remain unknown, but micro-movements of the fracture fragments are significant to a developing healing callus [2-3]. This study uses Finite Element analysis to evaluate an idealized neck fracture callus, for single, and two lag screw intra...

Potential Role of Local Estrogen in Enhancement of Fracture Healing: Preclinical Study in Rabbits

Background: Effects of estrogen on bone metabolism and its protective role on prevention of osteoporosis are well   documented. However, the efficacy of estrogen treatment on bone healing is not well investigated. The drug can be delivered both systemically or locally to the bone with differences in concentrations and side effects. The aim of this study was to investigate the effect of lo...

Stabilization of distal femur fractures with intramedullary nails and locking plates: differences in callus formation.

OBJECTIVES This study compared callus formation in distal femur fractures stabilized with locking plates and intramedullary nails to test the hypothesis that locking plates induce less fracture callus than IM nails. DESIGN Retrospective case matched study. SETTING Two orthopaedic trauma centers. PATIENTS 174 distal femur fracture were reviewed to extract cases treated with retrograde IM n...

Elastic Stable Intramedullary Nailing of Femoral Shaft Fracture-Experience in 48 Children

Background: Femoral shaft fractures are an incapacitating pediatric injury accounting for 1.6% of all pediatric bonyinjuries. Management of these fractures is largely directed by age, fracture pattern, associated injuries, built of the childand socioeconomic status of the family. We retrospectively evaluated the use of elastic stable intramedullary nail (ESIN)in surgical management of femoral s...
